Application Criteria
Please read ALL of the following criteria before filling out the application. If the application does not meet al the following requirements it will NOT be accepted.

DO NOT COMPLETE THIS APPLICATION UNTIL YOU HAVE BEEN INSTRUCTED TO BY A SPONSORING AGENCY, POST, OR SATELLITE POST ACADEMY.

Basic Application (first seven pages)

• Official Application is only found on this web site.
• Application must be typed - NO HANDWRITTEN applications.
• By using the application on this web site you can type in or check your answers and print out the application.
• All questions must be answered. If the question does not apply type 'NA' for not applicable. If there is no middle name type 'NMN' for no middle name.
• Any "yes" questions must have a detailed written (typed) explanation attached that includes dates, times, locations, people involved etc.
• Residence and work histories must be typed.
• Last Page should be signed by agency head and notarized. notary must fill out the county and state area.
• NO COPIES - all pages must be the originals.

Photograph

• Photo cannot be older than six months prior to making the application.
• No larger than 2"x2" and attached to page 7 in designated area.
• Best place for the photo to be taken is from a photo studio at Wal-Mart, K-Mart, Sears, or Kinkos.

Medical Release

• The original medical release must be signed by both the applicant and the physician.
• NO COPIES - Original document must be supplied with the application.
• The release must be completed within six months prior to the application be submitted.

Application For Certification

• POST and FHA applicants must have the form filled out and signed by and agency head, Chief, or Sheriff.
• NO COPIES - Original document must be supplied with the application.

Driving Record

• Driving record should be no older than 2 months prior to submitting the application.
• NO COPIES - Original document must be supplied with the application.

Waiver of Liability

• Signed by applicant in the presence of a notary - Dates Must Match.
• Notary must fill out the 'state of' and 'county of' portion.
• NO COPIES - Original document must be supplied with the application.

Authorization to Release Information

• Signed by applicant in the presence of a notary - Dates Must Match.
• Notary must fill out the 'state of' and 'county of' portion.
• NO COPIES - Original document must be supplied with the application.

High School Diploma/GED

• A copy of diploma, official transcript showing graduation date, or GED certificate is acceptable.

Birth Certificate

• A copy of birth certificate must be included with the application.

Military DD-214

• If applicant served in the military a copy of a DD-214 must be included.
• DD-214 -- must show the type of discharge.

Entry Test Scores

• A copy of the test results of the Utah Peace Officer Entrance Exam should be included.
• Results must show a passing score.

PDF Application

• Unless you have the full version (purchase required) you cannot save your entered information. Once you close the application all changed are lost. It is recommended you print out each page individually once completed.
• Any additional supporting explanations can typed on a word processor program. Ensure that your full name (as it appears on the original application), social security number, and date of application appear on the top of each page.
• The PDF file is in the Adobe Acrobat format. You will need the free program
  to view this file. The program can be downloaded at Adobe Web Site.
• The PDF version of the application expires every six months. Visit this web site to get the current application. Expired applications will not be accepted.
